These podcasts, excerpted from a March 2018 webinar, explore the value of the interdisciplinary team (IDT) in supporting and empowering participants with disabilities. A strong IDT is necessary for providing care coordination that keeps the participant’s experience and concerns at the forefront of care delivery. These conversations include practical and functional approaches to developing, managing, and executing IDTs. These discussions examine a highly integrated provider and plan IDT, while also exploring strategies to build and support partnerships with local community-based practices. Presenters provide concrete and innovative strategies to improve the management and communication within the IDT, including meeting execution, agenda setting, and facilitating communication when in-person meetings are not practical. Refining or designing new IDT processes/procedures is a crucial step to improving the participant experience and promoting participant-centered care.
